I work at local, regional, national and international levels to develop innovative models of collaborative community based services for individuals with complex care needs in order to transform patient outcomes and experience. These include: general practice based care for psychosis; world leading mental health care for prison leavers, being taken up by the West Midlands Mental Health Commission; and the regional whole system initiative for bringing together redesign, evaluation and research for patients with complex multi-morbidity. 2. I have 22 years‘ experience as a GP and now work primarily as an ‘outreach‘ GP for homeless or marginalised groups, supporting individuals with mixed mental, physical and addiction problems to navigate complicated NHS services. 3. As an accredited GP with Special Interest in Mental Health (since 2006) I helped establish and continue to work in and develop the Icebreak service for emerging personality disorder providing the only medical input. 4. As Professor in Primary Care Research, Deputy Director of the Peninsula Collaboration for Leadership in Applied Health Research and lead for the Plymouth University Primary Care Group I have led five major research projects with a total value of £3.3M, been a co-applicant on seven further NIHR grants totalling £18M and developed the group from 16 to 44 staff. 2 ACCEA BRONZE AWARD 2017 Name: TIM COLEMAN GMC/GDC Number: 3301209 Speciality: ACADEMIC GP I held a Bronze award from 2012-17; a 2016 renewal application was 2.8 pts below the required level so I now present this application. Since 2012, I have made national-level contributions to the NHS by: NHS research: being an international leader of research into ways of helping pregnant women stop smoking, publishing world-class study findings in the highest impact journals, leading two substantial NHS programmes and three major NHS randomised controlled trials; all work is implemented in partnership with NHS patients and is informed by their views NHS research training: leading Nottingham‘s GP Clinical Academic Programme and overseeing a substantial expansion in regional NHS general practice research training thus sustaining the delivery of effective, primary care-led NHS research into the future NHS healthcare provision: improving health & reducing inequalities by implementing novel NHS innovations for engaging pregnant smokers in treatment and helping them to stop, particularly focussing on finding effective methods to reach those women who would otherwise not use and hence benefit from NHS cessation support Translating research into NHS practice: my research outputs informing health policy internationally, underpinning World Health Organisation & NICE guidance on smoking in pregnancy and after childbirth 3 ACCEA BRONZE AWARD 2017 Name: ALEXANDER HARDING GMC/GDC Number: 3590630 Speciality: ACADEMIC GP First I design and implement educational innovations locally and nationally: • Helping to develop and implement 2 new medical schools (2002, 2012) • Introducing a new innovative multi-professional Masters programme (2016) • Introducing the UKs first substantial longitudinal learning programme (2016) • Introducing the UKs first joint undergraduate and postgraduate quality assurance visit programme (2012) • Leading national policy formation: Working with press (Times 2016), meeting with MPs (Exeter, Bradford and Totnes, 2015), writing parliamentary questions (Hansard 2015), informing health select committees (2015) and subsequent government reports. Subsequently named as one of the UKs most influential GPs (Pulse 2016). • Negotiating sustainable funding for GP educators: Increasing funding for GP education by £2M per year in the South-West England and writing the formula for the national tariff for GP teaching. Chair (equivalent of President) of RCGP - Leading the Profession: The most senior elected office, policy and political lead for the largest Royal College (52,000 members) at a time of huge challenge and inevitable tough decisions. I have improved communication with GPs and government by building relationships, refusing to be intimidated and being honest. I have appeared on >50 TV new & review programs and given >100 radio interviews, my priorities are workload (patient safety), workforce (building) and interface of primary/secondary care 2. Vice Chair Academy of Medical Royal Colleges: I am using my influence constructively to encourage all Royal Colleges to unite and campaign with one voice and steer government more effectively. A great collaborative spirit already building 3. Assessment of the GP Forward View: I led an in-depth assessment of the implementation of the promises by NHS England and HEE including a review of the 44 STPs and identified weaknesses and opportunities which have already been fed in to revised planning and improved communications 4. Increasing Recruitment to General Practice nationally: By influencing change in medical schools, in RCGP and working with HEE closely. Delivering numerous inspirational talks and keynote speeches with excellent feedback about my positive impact on trainees choice of future career 5 ACCEA BRONZE AWARD 2017 Name: JOSE MARIA VALDERAS MARTINEZ GMC/GDC Number: 6159785 Speciality: ACADEMIC GP • Direct patient care St Leonard's Practice (2 PA sessions, research group), practice rated as OUTSTANDING (Care Quality Commission 2016): "internationally recognised lead for learning, research and improvement" • Patient Safety: developed & implemented patient reported tool on safety experiences and outcomes in General Practice, endorsed by Royal College GP (2015); Patient Engagement Lead for WHO Safer Primary Care (2014/6); Quality/Safety Expert Advisor to WHO programme Non Communicable Diseases (2015/7) • Multimorbidity: NIHR funded programme (2011/6 £1M) for use of Patient Reported Outcome Measures (PROMs) in patients with multimorbidity, identified as priority area by NIHR (2014 and 2017 calls)/NICE (2016 guidelines); work cited in 2016 NICE guidance (1) and RCGP multimorbidity vision report (3); invited by European Commission to advise on priorities (2015) Evidence of quality of contributions through sustained clinical academic progression (Manchester, Oxford, Exeter) supported by competitive NIHR personal awards (Lecturer (2010/4), Clinician Scientist (2011/6)), and progression to Clinical Professor (2013-present), Exeter (top 10/126 UK universities, Times Higher Education 2016); First GP to be elected by peers as President of the International Society for Quality of Life Research (2016- 2020) 6 ACCEA BRONZE AWARD 2017 Name: MAURIZIO CECCONI GMC/GDC Number: 6118600 Speciality: ANAESTHETICS • (14-17) I have been an executive committee member at the European Society of Intensive Care Medicine (ESICM, 8000 + members) where I developed an online educational platform that delivered free education worldwide and as Chair of Scientific Affairs I fostered international collaboration, expanding the scope of the Society and organising the first European Asian Conference. • (15-16) I restructured a failing Critical Care Department at a nearby hospital. A CQC inspection identified problems with leadership, team dynamics, quality and patients safety. I improved the service by implementing a robust clinical governance, led a change in culture and improved quality. This was testified by recent positive external peer reviews. • I have one of the highest research outputs in anaesthesia, intensive care and perioperative medicine (3267 citations, h index 26). I have been promoted to Reader and received a commendation for a research programme at Harvard Medical School. • (14-17) As director for CRN South London Division 6 I have more than doubled the number of patients enrolled in NIHR studies, with more than 20000 patients enrolled during my mandate. I have secured funding for the development of a team of research nurses that has been consistently among the highest national recruiters. 7 ACCEA BRONZE AWARD 2017 Name: JOSEPH COSGROVE GMC/GDC Number: 3303713 Speciality: ANAESTHETICS For 16-years I have ensured my NHS Trust (rated Outstanding by CQC) & specialty have high national & international profiles. I jointly run one of the busiest intensive care units in England (>1300 admissions/yr) & have significant national & international roles in Medical Education (Royal Colleges of Anaesthesia & Surgery), Events Medicine, Major Incident Planning & Advanced Care Planning. Four examples of high quality work since my last award are: 1. Revision & redevelopment of regional Do Not Attempt CPR (DNACPR) charts (2014), minimising futile treatments across multiple care areas. My direct day to day work in making such decisions added significant credibility to the document 2. Reducing Health Care Associated Infections (HCAIs) in intensive care (2014- 2017) particularly central venous catheter infection rates: consistently < 0.25/1000 catheter days (national target 1/1000) 3. Redeveloping the CCrISP (Care of the Critically Ill Surgical Patient) Course (Fourth Edition, 2017) & delivering it internationally for the RCS England 4. Establishing and chairing a national voluntary, multi-specialty national working party (Nov. 2014) two years in advance of the conclusion of the 2016 Hillsborough Enquiry. It highlights requirements for Events Medicine & pre- empted Qu. 13 of the Enquiry (emergency response & medical cover) 8 ACCEA BRONZE AWARD 2017 Name: FIONA DONALD GMC/GDC Number: 3057603 Speciality: ANAESTHETICS I am an elected member of Council of the Royal College of Anaesthetists which is the 3rd largest Royal College with 21500 members & represents the largest single hospital specialty. In this role I work to: • Assure quality of patient care by maintenance of professional standards in anaesthesia • Shape national policy on health & medical training • Support the College‘s strategic direction I am a senior examiner for the Final Fellowship of the Royal College of Anaesthetists which is taken by 1000 candidates/yr: • I have been appointed to leadership roles in recognition of my sustained contribution to development of the exam • I am leading colleagues & working with educational experts to introduce a new form of written question with the aim of further improving exam reliability • My examining style is described as exemplary I demonstrate excellence in my role as a practising obstetric anaesthetist & in development of our service: • My complication rates are substantially lower than national standards & I have excellent patient feedback • I am a key member of the team developing & delivering evidence based training that is linked to improved outcomes in the UK & > 70 countries worldwide • Our unit was recognised as an exemplar of good practice for this training system both in the 2016 National Maternity Review & by the CQC 9 ACCEA BRONZE AWARD 2017 Name: THOMAS GALLACHER GMC/GDC Number: 3260771 Speciality: ANAESTHETICS Intensive Care Medicine is the newest CCT programme in the UK. I led the team who designed and implemented its recruitment process. It was necessary to allow for entry from 3 core training streams and 5 partner CCT programmes from 3 partner Colleges. It is the most complex system in the UK and required my liaison with partner Colleges and Deans to facilitate it‘s introduction. I increased the number of posts from 72 to 158 over 5 years whilst maintaining a 90% fill rate. For this, I received a commendation from FICM Board (2011-16). As Medical Secretary for FICM I approve CCT final sign offs, review all new training post applications as well as prospectively approving trainees out of programme training (2012-). In response to the Francis report recommendations, I introduced a weekly online quality assurance questionnaire for Foundation trainees. Completed weekly by each trainee it evaluates the quality of training, patient safety and undermining in real time. The latter 2 issues are addressed within 24 hours by means of automatic emails to the senior education team. This was commended by CQC in the well led category for which my Trust received an outstanding rating. As a member of the GMC‘s Project Action Group for Dual CCT Training, I advised on the standards for dual CCT programmes which were implemented by the GMC in 2015. 10
