The 9-month-old you admitted with dehydration and vomiting is a former 28-week infant who was thriving…until tonight. You examine the abdomen once more, re- look at the X-ray, see the rise in the lactate, and now know you have crossed over the medical chasm—you need a pediatric surgeon. As a pediatric critical care physician, I am fortunate to know unique individuals who have mastered the discipline of pediatric surgery—those physicians who can easily engage and comfort a distressed child (and family) and can adeptly diagnose, repair, and/or remove the source of a child’s distress. The editors are all close friends and colleagues. They are kind, patient-centered, and highly accomplished surgeons with over 75 years of cumulative experience. I have been elbow-to-elbow with each of these surgeons on many a February night. However, I wondered why four of the busiest people I know would take on a textbook. Then I was struck by a line in the preface of the first edition “Every time a pediatrician diagnoses a child with abdominal pain, an emergency medicine phy- sician cleans a child’s wound, or a nurse chooses a location for an intravenous line in an infant, pediatric surgical care is being delivered”. I believe this summarizes the intent of the editors—a desire to educate providers across skill sets so to optimize the care of any infant or child with a potential surgi- cal disorder. This is not surprising and reflects the generosity, lack of hubris and the expertise these thoughtful editors demonstrate day in and day out…nights included. Injury and surgical illness can result in critical con- ditions in children requiring rapid assessment and action to achieve a return to health. It is important to remember when there is a sick child, there is a sick family, and the needs of parents must be addressed. Pediatric surgery as a field is challeng- ing due to its breadth. It remains a generalized discipline in which the conditions treated span literally head to toe. It is oft repeated that children are not small adults. The surgical conditions of children are distinct from adults, and signs and symp- toms in children indicate different etiologies. A relatively small number of pediatric surgeons are concentrated in tertiary treatment centers so children with surgical disease will often present to non-pediatric specific providers. This text is designed to assist those caring for children whether they are on a pediatric surgery team or not. It is organized by specific surgical diseases affecting children at each stage of life, as well as chapters on trauma, neoplasm, and technical procedures. Chapters organize information in outline form to enable rapid diagnosis and treatment. Chapter structure includes key points, a narrative section of addi- tional details, study questions, and further reading. Each topic is written to stand alone for rapid reference, or the text can be used as a whole for study. We editors have devoted our professional lives to improving the lives of children and hope that this work helps others help children in need.
