State agrees to use data for surveillance or prevention purposes only Not for any regulatory or punitive actions Not for public reporting of institution-identifiable data CDC/NHSN will provide a minimum of three months after the agreement is signed for facilities to opt out of voluntary reporting to NHSN before data will be shared.
