Phone: (812) 738-4124 E mail: [email protected]. • Project involves general contractor services for the renovation of a building into 7 apartments, 3 one bedroom and 4 two bedroom and new construction of two duplexes consisting of approximately 2000 sq. ft. each. Each apartment in the duplexes will consist of two bedrooms. • The current building, a former childcare, will need to have interior walls demolished. • Project is funded with public and private dollars. • The project will NOT be required to pay Davis Bacon prevailing wage. Pre-Bid Conference A Pre-Bid Conference will be held on at 9:00 a.m. at Salem Childcare located at 202 Westminster Drive, Salem, IN 47167 on Tuesday, January 24, 2017 at 9:00 a.m. Conference is mandatory for firms wishing to bid. Bidders will be permitted to tour the building after the conference. Required Services Blue River Services, Inc. is requesting bids from qualified general contractors. Required services include: 1. Ensure proper materials are ordered and installed per construction documents. 2. Ensure all project submittals are provided by subcontractors and approved by project architect. 3. Coordinate all project scheduling of subcontractors, material deliveries, and sequence of construction and assignment of space where sub-contractors are performing work. This includes supervision of site work. 4. Develop and implement procedures for review and certification of sub-contractors’ applications for payment. 5. Submit a Project Application for Payment on a monthly basis. 6. Obtain waivers on a monthly basis. 7. Provide proper certificates of insurance and provide copies to owner. (See attached insurance requirements.) 8. Provide on-site project supervisions of sub-contractors and ensure all work is performed in accordance with Construction documents, local, state and federal building codes, and all safety and other regulations that apply to construction projects. 9. Coordinate the correction and completion of all work. The work for which bids are requested includes all labor, materials, equipment and items of materials and services required to provide the proper and complete construction of this project. The work shall be constructed in accordance with this request to bidders, project manual and drawings as prepared by John Hawkins, Architect. In order to facilitate the timely completion of this project, Blue River Services, Inc. expects to select a General Contractor on or about March 2, 2017. Bid Contents Complete proposals will contain the following information: 1. Qualifications of the Respondent List of Sub contractors by trade. 2. Detailed description of the scope of services to be provided. 3. Description of similar project experience. 4. Resumes of key project staff. 5. Proposed timeline for completion. 6. Fees 7. References 8. Federal Tax Identification Number 9. Bid Security in the amount of five percent (5%) of the Proposal, including all bid alternates must accompany each proposal in accordance with the instructions to Bidders See attached insurance and bonding requirements. 10. Bid shall be executed on Bid Form 00303, with all sections completely filled in. 11. All price quotes are to be free of all federal excise taxes and state sales tax, and upon request of supplier, tax exemption certificate will be executed by the owner to the successful bidder to cover all items subject to this tax. Contractor receiving award shall furnish an approved performance bond, labor and material payment bond for 100% of the contract which shall be in full force and effect for 12 months from date of acceptance of this work. Bond shall be furnished at time of written notice to proceed with the work. See attached Insurance Requirements. Blue River Services, Inc. reserves the right to: (1) accept any bids by a letter of intent within 60 days after the time of bid opening and a presentation of a form of agreement within 30 days from the date of such letter of intent; (2) at any time reject any and all bids; and (3) to waive informalities, irregularities, or errors in the bidding, provided it is not in violation of the notice to bidders, the statutes of the State of Indiana or Office of Management and Budget (OMB) Circular A-102, Attachments B & O and OMB Circular A-87. The owner shall have the option of including or rejecting any and or all alternates within 60 days from the date of the form of agreement between owner and contractor. Drawings & Specifications All bidders will have free access to a complete electronic set of Drawings and Specifications. All bid documents may be downloaded free of charge in electronic PDF format for viewing, printing and distribution to bidders, sub-bidders, suppliers, and reprographics services at the discretion and responsibility of the General Contractors. Bidders shall complete the Plan Holder List form via http://www.koverthawkins.com/bid-information Upon completion of the form, bidders will be re-directed to the Project Page where all bid information may be downloaded. Bidders should bookmark this link for future access. A list of updated Plan Holders and Addenda will periodically be posted and made available for download. Bidders are responsible for costs of any desired printing of drawings and specifications. The Architect retains all copyright to the bid documents as instruments of their professional service. Bidders or any other persons may not use the PDF files for any other purpose than preparing a bid for this project. All General Contractors planning to submit a bid for this project are required to be Registered Plan Holders. Registered Plan Holders are only those who complete the Plan Holder List form via the Architect’s website as indicated above. Addenda and any other additional information will be emailed only to these registered plan holders (using the address provided on the Plan Holder List form) as they become available. Bidders obtaining partial copies of the bid documents from any other source are not Registered Plan Holders and will not be automatically provided with Addenda or other bidding updates as prepared by the Architect. Non-Registered Plan Holders assume all responsibility for obtaining all necessary information in a timely manner. General Contractors shall certify on the Proposal Form that they have obtained a complete set of construction documents, including all Drawings, Specifications and Addenda, and have reviewed the jobsite to sufficiently familiarize themselves with the existing conditions. All bidders must submit a non-collusion affidavit stating that the bidder has not entered into a combination of agreement relative to the price to be bid by a person; to prevent a person from bidding; or to induce a person to refrain from bidding; and that the bidder’s bid is made without reference to any other bid. Bidders must submit evidence per Indiana Statute that all plumbing work will be performed by a licensed plumbing contractor. Conditional or qualified bids will not be accepted. Each bidder is responsible for inspecting the project site and for reading and being thoroughly familiar with the contract documents and specifications. The failure or omission of any bidder to do any of the foregoing shall in no way relieve any bidder from any obligations with respect to its bids. Review Criteria Blue River Services, Inc. will review all proposals submitted on the following criteria: • Experience of the firm and its personnel with similar construction projects. • Capacity of the firm to complete the project within stated time limitations. • Price. • Preference for local firms. • MBE/WBE firms encouraged to apply. Blue River Services, Inc. will select the respondent with the best qualifications for the project. Interviews may be held and additional information may be requested.
