living with HIV/AIDS to live healthier, longer, more independent lives. We address clients’ issues within a community that is responsive and caring and thus postpones or obviates the need for long-term hospitalization or residential health care. The goals of the ADHCs include the elimination of barriers to care, improvement in treatment adherence, and reduced hospitalizations. Our team strives to provide sensitive, compassionate, integrated care in a professional atmosphere. The AHDCs provide a community for traditionally marginalized individuals with minimal bio psychosocial supports outside of the ADHC Programs. The Adult Day Health Care model is premised on the hypothesis that it can level the playing field and render HIV treatment for our clients as effectively as treatment delivered to the general population. This is achieved by keeping our clients as healthy as possible while they are living in independent settings. (Good dental care is key to maintaining the health of people with HIV/AIDS.) Our clients requested a full-service dental clinic that was easy to access for historically underserved and special needs population. In August 2003, through a partnership with Columbia University, Harlem United opened a state-of-the-art dental facility which offers a variety of comprehensive services. Eligibility Criteria: Clients must be18 years or older ADAP and Some Medicaid managed care plans are not accepted.
