MichaelJ.Aminoff Franc¸oisBoller DickF.Swaab Preface Withthecurrentepidemicofobesityanddiabetes,theprevalenceandconsequencesofdiabeticneuropathyaresetto increaseinboththedevelopedandthedevelopingworld.Theprevalenceofdiabeticneuropathyrangesfrom50%to 90%andmanifestsaspainfuldiabeticneuropathyinabout20%,erectiledysfunctioninabout40%,andfootulcer- ationin about 25%ofdiabetic patients.TherearecurrentlynoFDA approved therapies to prevent orrepair nerve damage,andonlythreeapprovedtherapiesforpainfuldiabeticneuropathyandanumberofnewtherapiesforerectile dysfunction. Given that approximately 33% of the direct costs for treatment of diabetes and its complications are attributedtodiabeticneuropathyandfootulceration,thisdemandsfurtherinvestmentforresearchinthisarea. Moderncentralandperipheralneuroimagingtechniqueshavesignificantlyimprovedourabilitytodiagnoseand monitortheprogressionofdiabeticneuropathy.Furthermore,molecularbiologyandtheomicsplatformtechnologies haverevealednovelpathwaysforthedevelopmentandtreatmentofdiabeticneuropathy.However,thetranslational impactofthesediscoverieswillonlyberealizedwhenoptimalsurrogateend-pointshavebeendeployedinclinicaltrials ofdiabeticneuropathy.
