CommunityHealthNeedsAssessment In2007,JohnSnowInc.(JSI)wascontractedbyBeverlyHospitaltoconductacomprehensiveneedsassessmentthatwouldidentifythe majorconcernsandprioritiesontheNorthShoresothatBeverlyHospitalcoulddevelopcommunityhealthprogrammingandservices thatwouldmoreeffectivelymeettheneedsofthecommunity. TheassessmentprocessisintendedtoinformBeverlyHospital’sfive-year strategicplantoensurethatitsservicesandcommunityhealthprogramsremainresponsivetothecommunitiestheyserveandthatstrong partnershipsarebuiltandrenewedwiththecommunitiesandotherstakeholdersinthearea. Thefollowingisasummaryoftheprocess andmethodsusedduringthethreephaseassessment. PhaseI– PreliminaryNeedsAssessment  Conductapreliminaryneedsassessmentthatreliesonpubliclyavailabledata.  Conductaseriesofstructuredkeyinformantinterviewsinternallywithselectstaffandboardmembers. PhaseII– TargetedCommunityEngagement  Conductaseriesofstructuredkeyinformantinterviewsexternallywithcommunityleaders,healthandsocialserviceproviders andotherkeystakeholdersinthecommunity.  Collectprimarydatadirectlyfromthecommunityresidentsthroughaseriesofdirectmailsurveys.  DevelopafinalNeedsAssessmentReportandconductastrategicplanningretreat. PhaseIII –StrategicPlanDevelopmentandReporting  ConsolidatealloftheprojectfindingsanddeliverablesintoacomprehensiveCommunityNeedsAssessmentandCommunity EngagementStrategicPlanthatwillinclude: o Alistofthecommunityhealthcareneedsandprioritiesidentifiedoverallandbythecommunity. o Supportingdocumentationanddata. o Recommendingpriorityhealthneedsandasetofpreliminaryprogrammaticrecommendationsforreview.  Developareportingstrategy;developaseriesofpresentations/materials;presenttothecommunity. CommunityBenefitsPlan Asmentionedearlier,theresultsofthesurveyandtheworkwithlocalcommunitygroupsindicatedfourthemesforhealthycommunity intervention: 1. LifestyleBehaviorModification 2. MentalHealth 3. ChronicDiseaseManagement 4. AccesstoHealthcareServices Todate,thisorganizationcontinuestopursueprogramsthatfocusonthosefourthemesinourcommunityoutreachandinCommunity Benefitsprograms(seeSection(s)5and8). Short-termgoals:  Screeningsweredevelopedinthefollowingareas:skincancerscreenings,oralcancerscreenings,papsmearclinics,depression seminars,diabetesscreenings,bonedensityscreenings,bloodpressureclinics,fluclinics,CPRprograms,cardiovascularrisk assessment,osteoporosisriskassessment,diabetesriskassessment,bodymassindexandbreastcancerriskassessment.  Anumberofdiseasemanagementinitiativeshavebeeninstitutedincludingcardiacrehabilitation,heartfailuremanagement, pulmonaryrehabilitation,osteoporosismanagement,vascularhealthandwomen’shealthscreenings. In2003,Northeast HospitalCorporation(NHC)createdtheLifestyleManagementInstitute(LMI).TheLMIprovidesprogramsandeducation,and proactivelyidentifiespopulationswith,oratriskof,establishedmedicalconditions. TheLMIoffersafullrangeofservicesto thecommunity,includingriskassessment,preventioneducation,diagnostictesting,coordinatedmedicaltreatmentand continuousmonitoring. Effectivediseasemanagementusingappropriatemedicalprotocolsreducesthenumberofhospital admissionsandemergencyroomvisits,shortensthelengthofhospitalstaysandimprovestheoverallhealthandqualityoflife forpeoplewithchronicillness. Withineachofthechronicdiseaseareas,aninitiativewasdevelopedtoidentifyandmanagethetreatmentofeachpatientand family. AdatabasehasbeendevelopedbytheLMIforpatientmaintenanceandappropriatefollow-up. KeyAccomplishmentsofReportingYear Byofferingavarietyofhealthandwellnessprograms,AddisonGilbertHospital,BeverlyHospitalandBeverlyHospitalatDanvers providespatients,familiesandcommunitymemberstheopportunitytoremainhealthyandwell. Manyprogramsareprovidedforfreeor foranominalfeeduring FY2009andtheyincluded,butwerenotlimitedto: CommunityHealthandEducationalFairs;AdultCPR& Baby/ChildSaverCPR;Vascular(PVD)Screenings;BloodPressureClinics;BoneDensityScreenings;DiabetesScreenings;Foot Screenings;FluClinic;SeniorAdultLuncheonLectures;OsteoporosisPrevention&Exercise;PrenatalYoga;SkinCancerScreenings; HearingScreenings;SpeechandLanguageScreeningsforChildren;andeducationalprogramsincorrelationtohealthyinitiatives. PlansforNextReportingYear AnticipatedGoalsandProgramInitiatives 1. Worktowardsbuildingprogramsthatstemfromtheresultsofthecommunityhealthneedsassessment. 2. Continuetocultivatenewcommunityrelationships,aswellasmaintaincurrentrelationshipswithcommunityorganizations, membersandbusinesses. 3. Createadditionalcommunity-basedhealthoutreachprograms. 4. Partnerwithlocalcommunitiesonmedicationtakebackprograms. 5. ContinuewithhealthanddiseasemanagementprogramsthroughtheLMIwithinspecificidentifiedareas. 6. CommunityBenefits at NHC andits affiliates servemanycommunities, withneeds as diverse as theindividuals wholivewithinthem. Byprovidingtargetedhealthandeducationprograms that respond totheuniqueneeds of thesecommunities, NHCstrives toimprovethehealthofNorthShoreresidents. Ourcommunityoutreachmission –toprovidearearesidents theeducationandtools theyneedtoremainhealthyand well–is evidencedinthewide rangeofprograms weoffer. From children’s healthtocardiachealthtoseniorhealth,wearepositivelyaffecting thelives ofthoseindividuals and families whorelyonus toreceivethehighest qualityhealthcare,services and education. NHC consists ofAddison Gilbert Hospital andBeverlyHospital,which arefull service communityhospitals providingquality,patient-centeredcaretoNorthShoreresidents. Services includematernity,pediatrics, surgical, cardiology,orthopedics, as well as otherspecialties. Thehospitals boast amedical staff 641physicians, 2,721 employees (full-time,part-timeandperdiem), and 352volunteers. BeverlyHospitalat Danvers, anambulatory care centerlocatedinDanvers is also anintegral part ofNHC.Addison Gilbert Hospital,BeverlyHospitaland BeverlyHospital at Danvers aremembers ofNHC,whichis anintegratedhealthcaresystem comprisedofa networkofhospitals, behavioral health,long-term care andhumanservice affiliates offeringNorthShoreresidents general andspecializedmedical care. NHC’s primaryservice areaincludes theNorthShorecommunities of Beverly, Boxford,Danvers, Essex, Gloucester, Hamilton, Ipswich, Lynn,Manchester,Middleton,Peabody,Rockport,Salem,Swampscott, Topsfield andWenham. Strategies usedinCommunityBenefits healthactivities include prevention,earlydetection,earlyinterventionand long-term management. Healthissues addressed encompass the realms ofsafety,chronic disease,infectious diseaseandsubstanceabuse. Also includedwiththeCommunityBenefits MissionStatement is theMissionStatement of NHC.Thecorporate MissionStatement is foundedintheconcepts ofquality,caringandcommunity. NortheastHealth SystemMission Statement Northeast HealthSystem is anetworkofpeopleandcare givingorganizations joinedtogetherbya shared commitment toimprovethehealthoftheindividuals andcommunities weserve. Section2 Internal Oversight Internal Oversight - Managementof CommunityBenefits Program TheCommunityBenefits program is organizedbytheDepartment of ExternalAffairs ofNortheast Health System withtheresponsibilityforoverall management of theprogramcenteredwiththe ManagerofCommunity Relations. CommunityHealth Needs Assessment Information Source In2007,JohnSnow Inc. (JSI) was contractedby NHCtoconduct acomprehensiveneeds assessment that would identifythemajorconcerns andpriorities ontheNorthShoreso that NHCcoulddevelopcommunityhealth programmingandservices that wouldmoreeffectivelymeet theneeds ofthecommunity. The assessment process is intendedtoinform NHC’s five-yearstrategicplantoensurethat its services andcommunityhealthprograms remainresponsivetothe communities theyserveandthat strongpartnerships arebuilt andrenewed withthe communities andotherstakeholders inthe area. Thefollowingis asummaryoftheprocess andmethods used duringthethreephaseassessment.
