O f course, it is well known that Niek van Dijk is one of the world’s leading experts when it comes to treating ankle problems. In fact, this needs some correction … he is the leading expert. This is not fulsome praise. Just look at his numerous peer-reviewed publications and book chapters, and then con- sider all the patients that he has been treating, and with such success. Many of them are top-level athletes, whilst others are just people like you and me; people with ankle pain. T his book is Niek van Dijk’s life work. You might call it his legacy. It expounds what he calls ‘The Amsterdam Ankle Philosophy’, the idea that the arthroscope is actually used for surgery, and not just for telling us what needs to be done. First there is a non-invasive diagnosis, which is then followed by arthroscopic treatment. Niek van Dijk has done extensive scientifi c work in all these fi elds, as well as performing extensive surgery, and that is why his understanding is so profound. The book can be regarded as an operational-manual, which has numerous top-quality illustrations and step-by-step instructions, and covering a com- plete chain-of-events. The author starts with ankle-symptoms, moves on to anatomy and biomechanics, and then deals with radiography (including CT, probably the most valuable tool in this area) and the other diagnostic tests, and fi nally the treatment (mostly mini-invasive) and the possible outcomes. As well as ankle arthroscopy, he covers all endoscopic procedures for diag- nosing and treating hindfoot, tendon, ankle, subtalar and foot pathology. As such, he presents a complete overview of this advancing fi eld, and of his personal approach – The Amsterdam Approach – which is proven to produce fewer complications than the other open and arthroscopic methods. My own conclusions from the book are these: • That most ankle and hindfoot pathology can be treated by arthroscopy, and with excellent results. • That ankle arthroscopy should never be performed without a prior and proper diagnosis. • That pre-operative planning should determine the approach: whether ante- rior, posterior or combined. vii viii Foreword The book is accompanied by a free interactive website – The Amsterdam Foot and Ankle Platform ( www.ankleplatform.com) – where the most impor- tant operative techniques are demonstrated, using text, pictures and videos, with weekly updates and tips-and-tricks for each procedure. There is also an interactive part, by which cases can be submitted to an international expert panel. I n summary, this is a unique book, written by a master surgeon and researcher, and based on a unique philosophy. All ankle-surgeons should study it. My philosophy, based on more than 25 years of experience, is that the arthroscope is not used for diag- nosis, but as a tool to perform surgery. This means that a diagnosis must be established before the surgery. By means of preoperative planning, it has to be decided how to approach and treat the symptomatic lesion or lesions. The approach can be by means of a two-portal anterior approach, by means of a two-portal hindfoot approach, or by means of a combined anterior and poste- rior approach. In some cases, an open procedure with or without an osteot- omy gives the best access. It is always possible to decide for the correct approach preoperatively. T he content of this book can be considered as an operation manual. It is accompanied by a free accessible interactive website ( w ww.ankleplatform. com ) on which the most important operative techniques are shown in text, pictures, and videos, including weekly tips and tricks for each procedure. The interactive part concerns the possibility to submit cases to an international expert panel. For the opportunity to complete this book, I am grateful to a number of people.
